Community Health Department


Home Health Services are ordered by physicians and provided by nurses; physical, speech, and occupational therapists; and trained CNAs who assist with personal care and rehabilitation activities.

Services

Nurses provide:

  • Evaluation of care needed
  • Education
  • Treatments such as dressing changes and medication set up 
  • Facilitate communication between patient and doctor
  • Refer patients to community resources
 

Home Care Aides provide:
  • Assistance with personal care (baths, etc.)
  • Communication of changes to the nurses
  • Assistance with prescribed exercises and walking

Homemakers provide:

  • Light housekeeping
  • Essential shopping
  • Laundry Assistance 

Physical Therapists provide:

  • Help to regain maximum mobility and independence

Speech Therapists provide:

  • Assistance with communication problems in relation to speech and hearing

Occupational Therapists provide:

  • Assistance to patients in achieving independence and improving abilities

Eligibility Requirements

All individuals living in Iowa within a 30 mile radius of Floyd Valley Hospital who need health services in the home (regardless of age, sex, race, political and religious affiliation or physical and socioeconomic status)

Funding sources available include Medicare, Medicaid, private insurance, and grant funding from both the Iowa Department of Public Health and Siouxland Aging.  No one is denied care due to inability to pay for services.

  • Health Families
    Healthy Families is a research-based, voluntary, national program for families expecting a child (expecting or within 3 months of birth).  It is free to anyone in Plymouth County and is funded by NW Iowa Community Empowerment Funding.  Confidential home visits are made weekly.  The first three years of a child's life are the most important for brain development.  The goals of the program are to strengthen families and provide safe, nurturing environments in which children thrive.

  • Lifeline
    Lifeline is an emergency response service in which a machine in the home connects the subscriber's home phone and a waterproof personal help button that is worn.  If help is needed, they push their button activating the unit, transmitting a signal to Lifeline Central.  Based on the situation, a listed respondent and / or emergency personnel are contacted on the subscriber's behalf. There is a one-time installation fee and a monthly rental fee.   Lifeline is a service partially funded by the Floyd Valley Hospital Auxiliary.
