We want to make your visit as comfortable and easy as possible. The information below can help you prepare for your visit.

Clinic Hours

We are open from 8 AM to 5 PM, Monday through Friday with appointments starting at 8 AM.

What We Need to Know

When you call us for an appointment, we will ask if you have been our patient before. If you have not, we will ask for the following information:

• Name
• Address
• Birthday
• Two contact phone numbers

What to Bring to Your Appointment

• We ask that you bring any medical records to your first appointment (unless they have already been transferred).
• Bring a list of all medications you are taking or have recently stopped taking. You can bring the actual containers if possible.
• Your insurance card or information.
• Any demographic information you might not have memorized (person to call in case of an emergency, new address, etc.).
