Avon Clinic

Hours: Monday 1:30-5:00 pm; Wednesday 8:00 am to 5:00 pm; and Friday 8:00 am to 12:00 noon

Please arrive 10 minutes prior to your scheduled appointment to complete necessary paperwork.
Insurance
As a courtesy, we will submit your insurance claims for you. Please bring your insurance card with you to each visit. Accurate and up-to-date information will help us file your claims promptly.
