We want to make your visit positive and stress-free! The information below can help you schedule appointments and prepare you for your visit to Avera Medical Group Nephrology!
We are open from 8 AM to 5 PM, Monday through Friday — not open on holidays. If you need to reach a physician other than during business hours, please call (605) 322-7600 and you will be connected to our answering service which will contact the on-call physician to assist you in your care.
What We Need to Know
New patients will be referred to us by your Primary Care Physician who determines that you need to be seen by a nephrologist. We will ask your Primary Care Physician to fax or mail your pertinent medical records to us prior to your appointment so that we can be prepared for your clinic visit. A Patient Medical History form will be mailed to you to complete and bring with you to your appointment.
If you are calling for a follow-up appointment, we will ask you if you have had a change in address or telephone number.
At the time of your appointment, we will ask you to complete a Patient Information sheet which asks for pertinent information needed for billing purposes. If you are new to our clinic, we will also ask to make a copy of your driver’s license for identification purposes.
If you need to have blood drawn or urine collected, we normally schedule your lab visit one hour prior to your appointment so that the doctor has the results for your visit.
