Fees/Billing
Fees for professional services, laboratory, x-ray and other special services are based on the nature and extent of the services rendered. Any information you would like about our fees is available to you. Ask our office staff or Patient Account Representative.
If you have concerns about the overall cost of your care, we encourage you to discuss this with your provider. He/she will utilize appropriate staff to provide the information and address your concerns. All co-payments are due and payable at the time of service. Payment within 30 days of billing is considered prompt payment. A service charge is added to accounts more than 90 days old. If you receive care for a work-related injury, please notify the receptionist when you arrive for your appointment, and your physician during the appointment.
If you have any questions about your statement or insurance processing, please call the business office at 605-697-9539 or 1-800-658-5405. We will be happy to answer your questions.
Insurance
It is your responsibility to provide the business office with accurate insurance policy information. In most cases, Avera Brookings will submit claims to your insurance company. These include Avera Health Plans, Medicaid, Medicare, Blue Shield or DAKOTACARE. With some insurance carriers, you will receive an Insurance Claim Form with your clinic bill, to be submitted to your carrier. Ask at our insurance office for claim submission details on your coverage. Payment plans are available for those without insurance. All providers at Avera Brookings are participating with Medicare and Medicaid.
