Clinic Information

We are open from 8 AM to 5 PM, Monday through Friday with appointments starting at 8 AM. We are located at 6701 S. Minnesota Ave, south of the 69th St. and Minnesota Ave intersection.

What We Need to Know

When you call us for an appointment, we will ask if you have been our patient before. If you have not, we will ask for the following information:

• Name
• Address
• Birthday
• Two contact phone numbers
