Avera St. Luke’s Home Health National 'Agency of the Month'

Avera St. Luke’s Home Health was picked as the national “Agency of the Month,” for September 2010 by Home Health Quality Improvement (HHQI), which is part of a national quality improvement initiative led by the Centers for Medicare & Medicaid Services.

Avera St. Luke’s Home Health submitted work done to achieve the 90th percentile for two categories, Acute Care Hospitalizations and Oral Medication Management. 

Avera St. Luke’s Hospital’s name and logo can be found on the Home Health Quality Improvement website, www.homehealthquality.org, under Agency of the Month. This information on Avera St. Luke’s Home Health was included:

“Our success is from our team working as one. It’s imperative to me that we involve all of our staff when improving the quality of care we provide. Each staff member serves on a specific committee for each of the home health OBQI Outcomes. They review our current process and decide if a change is needed to achieve best practice. If so, the committee identifies an action plan that addresses best practice. We must have those providing front-line care in the field tell us what is needed to improve the quality of care we provide to our patients.” Cindy Senger, RN, MS, Director of Home and Community Services
