Osceola Community Health Services

Osceola Community Health Services provides a full range of health care, home care and social services for all ages. An individual plan of care is developed to meet each person's special needs.

Home-Based Nursing Care

  • Skilled assessment and teaching
  • Wound care, dressing changes
  • IV therapy
  • Catheter care
  • Medication teaching and assistance

Home Care Aides are trained to assist:

  • Personal care/bathing and dressing
  • Exercises/walking/transfers from bed
  • Essential shopping/meal preparation
  • Light housekeeping/laundry
  • Companionship/relief for caregiver
  • Child and adult protective services

Rehabilitation Therapies

  • Physical therapy
  • Occupational therapy
  • Speech therapy

Hospice

Hospice provides compassion and support for the terminally ill and their families, with care to uphold dignity and keep pain and other symptoms under control in the patient’s own home or home-like setting, as well as grief support.

Public Health Services

As a public health provider, Osceola Community Health Services is committed to promote, preserve and protect the health of our communities.

Services include:

  • Immunization clinic - every Thursday (appointments preferred.)
  • Wellness/blood pressure/glucose screenings
  • Communicable disease follow-up
  • Lifeline
  • Home medical equipment
  • Health education

Tobacco Prevention

The Northwest Iowa Tobacco Free Coalition (NWITFC) provides guidance and support for local tobacco control interventions and initiatives. This includes tobacco prevention among young people, reducing exposure to second-hand smoke, and encouraging youth and adults to quite tobacco use. To learn more, call 712-737-2971.

Foot Care

Osceola Community Health Service offers foot care for the elderly and/or disabled residents of Osceola County. You are welcome to attend any clinic in the county, just call ahead to make an appointment. Cost is $23. If possible, soak your feet before your appointment.

  • Sibley: Second Wednesday of each month at Osceola Community Health Services, 115 Cedar Lane, IA. 9 a.m. – 4 p.m. 712-754-4611
  • Ocheyedan: First Wednesday of every other month at Ocheyedan Senior Activity Center, 845 Main St. 1 – 3 p.m. 712-758-3182
  • Melvin: Fourth Wednesday every other month at Melvin Civic Center Fire Station. 12:30 – 3:30 p.m. 712-736-2163

Referrals

Referrals can be made to Osceola Community Health Services by medical professionals, social workers, family members, and clients. In-home evaluations are available free of charge. Clients need not be in the hospital for home care services to be initiated.

No person shall be excluded from participation, be denied benefits, or be otherwise subjected to discrimination on the grounds of race, creed, color, religion, political affiliation, national origin, sex, age, physical or mental disability, testing positive for human immuno-deficiency virus, or sexual orientation.

Payment may be covered by Medicare, Medicaid, Iowa Department of Public Health Grant Funds, Northwest Aging Association Grant, private insurance or private pay, or county funding.

Osceola Community Health Services is a Medicare certified agency. The agency is governed by the Board of Health and Osceola Community Hospital Board of Trustees. Our office would be pleased to assist you with determining your funding source and will submit your claims for services.
