Info About Scheduling Mammogram
If you are having any breast related symptoms please contact your health care providers office so the appropriate imaging can be ordered to best fit your needs.
Please have the following information available when calling to schedule your mammogram:
- Name
- Date of Birth
- Phone
- Address
- Physician's Names
- Location of previous breast imaging examinations
- Reason for the exam, if any breast related symptoms or problems
- Breast health history, including surgical history as well as family history of breast cancer
- Questions about medications may be asked for some specific tests
