We Provide and Complete All DOT Mandated Paperwork

All of the forms necessary for your exam will be provided at the time of your appointment. However, if you would like to speed up the process, you can print the DOT Physical form and bring to your appointment with your information and health history completed.

Same-day Verification Medical Certificate

At the conclusion of your physical exam, you will be given the motor carrier copy of the Medical Examiner’s Certificate to send to your employer.

When is a DOT Medical Certificate Required?

  • If a driver operates a motor vehicle with a gross vehicle weight rating (GVWR), gross combination weight rating (GCWR), gross vehicle weight (GCW), or gross combination weight (GCW) of 4,536 kilograms or more in interstate commerce
  • If a driver operates a motor vehicle designed or used to transport more than 15 passengers (including the driver) in interstate commerce
  • If a driver operates a motor vehicle designed or used to transport between 9 and 15 passengers, for direct compensation, beyond 75 air miles from the driver's normal work-reporting location, in interstate commerce
  • If they transport hazardous materials, in a quantity requiring placards, in interstate commerce

How often is a Medical Examination Required?

How often is a Medical Examination Required?

  • Medical certificates are valid for 2 years at a maximum

Exceptions:

  • Hypertension (high blood pressure) stable on treatment: 1 year
  • Heart disease: 1 year
  • New insulin and vision exemption program: 1 year
  • Driving in exempt intra-city zone: 1 year
  • If a medical condition requires more frequent monitoring, length of medical exam certification is determined by Medical Examiner.
