Immigration Physicals

If you or a loved one is an alien applying for an adjustment of status or Permanent Resident Status (Green Card), count on Avera to provide the necessary immigration physical and paperwork.

Darla Edinger, MD, AME, CMIE, MRO at Avera Occupational Medicine Mitchell is a designated Civil Surgeon for the U.S. Department of Homeland Security, U.S. Citizenship and Immigration Service (USCIS). This authorizes her to perform medical examinations and provide necessary immunizations for aliens applying for adjustment of status or Permanent Resident Status. She also performs examinations for citizenship.

Immigration Examination Requirements

The following tests are required by the U.S. Citizenship and Immigration Service (USCIS). A physical examination is required for persons of all ages. Additional exam requirements are determined by your age and immunization record.

An adult examination (15 years or older) typically includes:

  • Physical examination
  • Test for tuberculosis
  • Blood test for syphilis (RPR)
  • Immunizations depending on age and medical history

A pediatric examination (children 14 years of age or younger) typically requires:

  • Physical examination
  • Test for tuberculosis
  • Review and update of the pediatric immunizations

A tuberculin skin test will be performed at your first visit. You must return in two to three days for a check of the test. If you have had a positive tuberculosis skin test, you will need to have a chest X-ray to make sure you do not have active tuberculosis. This can be done at the clinic for an additional charge. If the TB test is negative, your medical examination can continue.

Once completed, the results of your examination will be sent to an Immigration Officer and may also be shared with health departments and other public health or cooperating medical authorities.

Required Immunizations

Certain immunizations are also required by the USCIS. If you don’t have acceptable proof that you have had the immunizations or the diseases themselves, an Avera provider can give the immunizations you need.

Adult immunizations may include:

  • Measles, mumps, rubella vaccine
  • Tetanus, diphtheria, pertussis vaccine
  • Varicella (chicken pox) vaccine
  • Hepatitis B vaccine
  • Seasonal influenza (during flu season for certain individuals)

Childhood immunizations, depending on age, may include:

  • Rotavirus vaccine
  • Measles, mumps, rubella vaccine
  • Tetanus, diphtheria, pertussis vaccine
  • Polio vaccine
  • Haemophilus Influenzae Type b vaccine
  • Hepatitis B vaccine
  • Hepatitis A vaccine
  • Pneumococcal vaccine

What To Bring To Your Appointment

To ensure a smooth and efficient appointment, please bring the following items:

  • Government issued picture identification, such as an unexpired passport, driver’s license or ID. School IDs are not acceptable.
  • For applicants under 14 years of age, USCIS will accept other proof of identity that shows name, date and place of birth, parents' full names, and any other identifying information about the applicant. Acceptable documents include birth certificates (with translations, if necessary) or affidavits.
  • Your medical records for any chronic or serious medical problem(s).
  • Your immunization records, including childhood records, if available (with translation, if necessary) or an affidavit.
  • Alien Registration number (A-Number) if you have one.
  • The most recent copy of the I-693 medical form if you have one. We can provide this, if needed.
  • Translator, if needed, must be present to sign forms.

If medical records are not in English please have them translated before your appointment. Applicants who do not speak English will be provided with a translation service at an additional cost. Please let us know if this is required when the appointment is scheduled.

Cost of Exams

Contact your local provider for the base examination fee. The TB skin test, blood test for syphilis, chest X-rays, immunizations and other blood or medical testing and translation services are all at an additional charge.

All expenses in connection with the examination must be paid at the time of the examination via VISA, MasterCard, American Express, debit card, cash, money order or check.
