Your Home Health Care Team
You and your doctor—who oversees your care—work with several Avera home health team members, including:
- Skilled nursing care staff – Assess your health and deliver a wide range of services, such as medication, IV services and nutrition support, diabetes education, blood draws, wound/ostomy/Foley catheter care, high-risk pregnancy and aftercare for mother and newborn
- Social workers – Assist you and your family members in coping with challenges related to illness, disability and terminal conditions; evaluate problems, needs and coping mechanisms; connect you with community agencies, education programs, counseling services and support resources
- Pharmacists – Deliver advice and support regarding medications
- Dietitians – Offer nutritional guidance and support
