Avera St. Luke’s Home Health offers private duty services for nursing and homemaking to meet your needs. Each caregiver is trained to provide individualized care with a personal touch. So just settle back into the comfort of your own home and we’ll do the rest. We’re equipped to provide just the right care for you.
Registered Nurses
Nurses provide assessments, patient and family education, medication management, insulin syringe set-up, patient care, physician follow-ups, and coordination of community resources (Charge: $55 per hour).
Homemakers/Aides
Homemakers/Aides offer personal care and homemaking services to patients and families to assist with housekeeping and personal care tasks:
- Assistance with feeding
- Bath visit (bathing, shampoo, dressing, and oral hygiene)
- Companion services
- Errands (mileage charged)
- Grocery shopping (mileage charged)
- Laundry
- Light housekeeping
- Meal preparation
What Charges Are Involved for Avera St. Luke's Home Health?
Initial set-up fee – $30
Weekdays (7 a.m. to 7 p.m.) – $20 per hour
Evenings (7 p.m. to 7 a.m.), weekends and holidays – $25 per hour
Hourly rates for all services in Aberdeen include travel time and time spent in the home.
How Can a Home Health Referral be Completed?
Referral to Avera St, Luke’s home health can be made by:
- Physicians
- Patients
- Patient’s family
- Friends
- Community agencies
The patient must be under the care of a physician who will write an order for home care staff. A patient must live in the program service area, which consists of the Aberdeen community and surrounding 40-mile area extending into North Dakota.
Patients living outside of the Avera St. Luke’s Home Health service area are referred to local home care agencies located near the patient’s home.
All patients referred to Home Health will have a pre-admission interview by an RN from Home Care to determine if Home Care is appropriate.
Services are provided to residents of Ellendale, ND, and the surrounding area.
