Our Palliative Care Consultation Team
The palliative care consultation team may be called by the patient’s physician to aid in managing difficult-to-control symptoms, such as pain or shortness of breath, or to assist patients and their families who are beginning to think about advanced care options.
The team works together to support the patient and family during hospitalization, while clearly communicating with those already providing medical care. Members include:
- A doctor who specializes in recommending medications or treatments to help relieve pain and symptoms
- A nurse with advanced training in life-threatening illness and coordinating care, who can help arrange for health care after leaving the hospital
- A social worker to assist with interpersonal issues, lifestyle changes, hospital discharge planning, and coordination of community resources
- A chaplain who offers spiritual and emotional support, counseling and coping strategies, according to your faith and individual needs
