The center, located on the hospital’s fourth floor, helps more than 70 percent of patients admitted with significant physical and cognitive problems return home. You’ll find a warm, inviting Activity/Dining Room, and a transitional apartment that enables patients and their caregiver the opportunity to practice and explore new skills in a safe environment.
