My 365 - Online Health Guideman on scale

The online health guide is available for all Avera Health Plans' fully insured and Avera MyPlan members.  Members have access through a secured portal to store health information along with tracking opportunities to live healthier lifestyles.

Wellness in the Workplace

Avera Health Plans' strategy towards a healthier workforce is to offer health care solutions that meet the challenges of rising costs and benefits. We have a designated health and wellness champion who will come to you.

Avera Health Plans provides the following health and wellness services — click on the tabs below to learn more:

Health Screenings

Free to all fully insured employer groups in Lincoln and Minnehaha counties and to groups larger than 50 employees in our service area. A basic on-site screening includes:

  • Blood pressure testing
  • Cholesterol and glucose panel
  • Height and weight
  • Individual review of results with a health care professional

NOTE: Flu shots are an optional benefit and can be arranged for large employer groups.

Health Coaching

Our health and wellness champion will incorporate wellness programs within the workplace to help employers create environments that support healthy behaviors. 

 

Employers can email WellnessCoach@AveraHealthPlans.com to schedule onsite brown-bag sessions with our health and wellness champion or health screenings for your employees.

Care Management

For members experiencing complex illness or injuries, licensed registered nurses help members maximize their benefits for the following:

  • High-risk obstetrics and neonatal ICU admissions
  • Severe burns or major limb amputations
  • Repeated inpatient admissions
  • High-cost cases, complex conditions and chronic diseases
