AveraChart Proxy Access

If you need to coordinate care for another person, such as for your child, proxy access lets you access another person’s health information as you need to for coordinating care. You can use proxy access to:

  • View your child’s or dependent’s health information and contact their doctor.
  • See health information for an adult patient unable to make decisions on their own.
  • Give another adult access to your AveraChart account.

To request proxy access, download the form and either return it to your clinic or fax it to 605-322-8223.

Please note:

  • Proxy access for minors is limited to parents and legal guardians.

You have the right to revoke or remove an authorization for access to your AveraChart.

Update Proxy Access in AveraChart

To check your AveraChart account to see if you have a proxy and manage those who have access, log in to AveraChart and follow these steps. You can also use this tip sheet to see how to remove proxy access from someone.

On a desktop device:

  • Select your Profile icon.
  • Click on Shared Access. This shows you all of the people who can access your account and the last time they accessed your medical information.
  • If you see someone on the list that you no longer want to have access to your health information, select Remove, and then OK.

On the MHealth mobile app:

  • In the app, tap the three lines menu on the top right of your screen to see your menu options.
  • Select Shared Access. This shows you all the people who have proxy access and when they last accessed your account.
  • To remove someone you no longer want to have proxy access, tap the Trash icon on the lower left side of the box on the page. Then tap OK.

Fill out the Proxy Revocation form and follow the instructions to return it to your local health facility.

I have proxy access for my child or loved one. How do I view their information?

When you’re logged into the portal, you should see a Change Person button in the upper right corner of the screen. When you click that button you will see additional records you can access.

How can I see who has access to my AveraChart?

From the home page, select the Profile button and click Shared Access. This shows you all of the people who can access your account and the last time they accessed your medical information.

I previously could see my teenager’s records but now I can’t access them. What do I do?

Patients under the age of 18 (19 in Nebraska) may not deny AveraChart access to a parent or guardian, with a few privacy exceptions. Certain things can be blocked from the chart – a privacy exception – that follows the Information Blocking rules already in place.

How do I change the default record that shows when I log in to my AveraChart?

From the portal Home page, select the Preferences icon, and then the Update User Preferences button. Select the down arrow and choose a patient from the list, then click Submit. This patient’s record will automatically be displayed first when you log in to AveraChart next time.
