Patient Information
New patients are asked to bring two forms of identification and insurance information. In the event you are not covered by medical insurance, you will be asked to provide a $50 prepayment.
It is the patient's responsibility to provide the business office with accurate insurance policy information. Please bring your insurance card, Medicaid or Medicare card to your appointment. We will submit claims to your insurance company as a courtesy; however, the account balance is ultimately the patient's responsibility.
