Home Health Care Services
Avera@Home offers a full range of skilled nursing, therapy and home health aide services. By providing direct care and education, our team can help restore and improve a patient's health status. Staff are experienced and trained in current nursing and clinical practices for both adult and pediatric patients.
Care Comes to You
Services may be short-term following an illness or surgery or long-term for patients with chronic diseases. If you or a loved one needs help with strength building and recovery, mobility, home safety or managing symptoms and chronic diseases, we work with your care team to find what’s right for you. Together we’ll help you get better while having all your needs met comfortably and conveniently at home.
Recover while having your needs met conveniently at home. Skilled nurses assess needs and develop a plan of care to help patients recover from illness or maintain health and safety, and provide services such as:
- Medication management to ensure medicines are working with a clear schedule
- Assessments to ensure the home living environment is safe
- Changing of bandages/dressings
- Wound/ostomy/Foley catheter care
- IV services
- Blood draws
- Diabetes education
- Care for high-risk pregnancy and after care for mother and newborn
In-Home Services
In-home caregivers provide companionship and help with bathing, grooming, personal hygiene, meal preparation, grocery shopping, light housekeeping, laundry, personal correspondence, and getting in and out of bed or chairs.
Home Therapy Services
Avera@Home services help you maintain your independence or regain strength and functionality with in-home physical, occupational and speech therapies. Therapists come to your home with the goal of getting you back to how life was before the health event.
Home Medical Equipment
Patients can set up appointments at numerous sites for consultations with our experts. Explore a variety of home health and medical equipment products to aid patients with special needs such as:

Hospice Care
Avera@Home offers specialized hospice services 24/7 for patients with a life-limiting illness, including both medical and emotional support, plus pain and symptom management. Our office team can help facilitate care and services for hospice in a variety of settings:
- In your home
- In a hospice residential facility, long-term care facility, assisted living centers and some hospitals
Holiday Hours
Our holiday hours may vary for major holidays, including New Year’s Day, Memorial Day, July 4th, Labor Day, Thanksgiving and Christmas.
