As a Level IV trauma and Comprehensive Advanced Life Support (CALS) designated hospital, Avera Granite Falls Health Center offers superior local inpatient, outpatient, emergency care, ambulance and specialty services such as surgery, radiology, mammography, obstetrics, home health, skilled nursing care, independent senior living and hospice care to people in our surrounding community.
Patients & Visitors
- Parking for emergency services is located on the street of 10th Avenue near the entrance with a sign marked “Emergency.” All other patients and visitors may park in the gravel parking lot to the west of the building.
- All non-emergency patients and visitors are welcome to enter through the main entrance under the big canopy and check in with reception just inside.
- Most general care areas welcome visitors during the day. Please keep in mind that doors to the facility lock after 5 PM for security purposes.
- We recommend no more than two visitors at a time. Minors and dependent adults must be accompanied by a responsible adult.
- We may restrict visitors, temporarily close and adjust hours to protect the safety of patients and staff.
- Find a quiet place for prayer or meditation on campus.
- Learn about your patient rights, responsibilities and role in safe health care.

- For written concerns or complaints, contact the hospital’s main phone number and ask to speak to the risk management department.

Mammography
Avera Granite Falls Health Center offers digital and 3D mammograms, an important health screening for women ages 40 and older. Request an appointment at this location or call 320-564-3111.

Radiology & Imaging
We offer innovative imaging technology, including X-rays, CT and DXA scans, digital and 3D mammography, nuclear medicine and ultrasound, which are available for clinical staff to order around the clock for patients making scheduled visits. We also offer MRIs at this location via mobile unit. Experts conduct comfortable and quick imaging, so you receive accurate diagnosis which will determine the right treatment for your health.

Laboratory
Right inside our facility, you have access to a wide range of laboratory services that contribute to accurate diagnoses of medical conditions. Expert specialists conduct safe and reliable lab testing, helping you and your provider get the results you need to start an effective and personalized treatment plan.

Avera Lab Direct Access Testing
This location also offers Avera Lab Direct Access Testing, which lets you quickly and confidentially order a variety of tests without a doctor’s order. These tests help you monitor ongoing risks — such as complete blood count, glucose, lipids or hemoglobin — but shouldn’t be a replacement for the personalized care of your primary care provider.

Ambulance Services
ALS Ambulance Service transfers acutely ill patients to and from Avera Granite Falls Health Center and surrounding health care facilities. We strive to provide safe and quality care which meets or exceeds the needs of the patient and their family.

Special Transportation Service
Our Special Transportation Service is managed and operated as a subdivision of our ALS Ambulance Service. Our vehicles are operated by staff committed to providing safe, timely and efficient transport.
We offer:
- Local or long-distance transportation
- Stretcher transport (additional attendant required)
- Door-to-door service
- Drivers who come to your room and help you to the transport vehicle and wait to take you back home from an appointment. Family members are welcome to ride along.
Special Transport Services operates 24 hours a day, seven days a week. It’s recommended that you call at least one day in advance to schedule your appointment. Call us at 320-564-6274.

The West Clinic is located on the west side of the Avera Granite Falls campus. Ample, free parking is available next to the building. Enter through the west door.
Cardiac Rehabilitation
Cardiac rehabilitation treatment sessions can help you recover from a heart attack, disease or surgery. Our skilled experts will help you make your heart as healthy as possible with safe, monitored exercise and education to modify your lifestyle, diet and health at home. With guidance from our team, you can:

- Regain your independence to live a full and active life
- Rebuild confidence in a supportive, professional space

Pulmonary Rehabilitation
Pulmonary rehabilitation treatment sessions help improve quality of life for those diagnosed with lung conditions. The care team evaluates and instructs patients on physical activity, nutrition, stress management and medication education, along with meeting other educational needs for patients.

Infusion Services and Treatments
Services focus on intravenous (IV) infusions of medication and treatments for conditions including:
- Infectious diseases, including COVID-19, using antibiotics and other medications
- Blood transfusions and plasma exchange
- Dehydration treatment
- Services for OB patients as ordered by their provider
- Electrolyte replacement therapies
- Rabies injections and treatment
- Treatment for various autoimmune disorders in specialty areas including gastrointestinal (GI), rheumatology, nephrology and neurology
Swing Bed Provider
This hospital is a swing bed provider under federal regulations, meaning that it can use hospital beds, as needed, to provide either acute or skilled nursing facility care. Post-hospital extended care services provided in a swing bed hospital are allowed coverage under Medicare Part A. This allows a patient to continue receiving services in the hospital even though acute care is no longer required.
