Yearly Checkups
Yearly checkups allow you and your primary care provider to build a trusted relationship around your health — whether that’s physical, mental or emotional health. During this annual doctor’s visit, you and your provider might discuss your:
- Current health status
- Immunizations
- Health history
- Potential health risks
- Preventive screenings you may need
- Personal health goals
- Current prescriptions, over-the-counter drugs, vitamins or minerals you’re taking
This appointment is also the time to bring up any concerns or changes you’ve noticed so that your provider can help you take the next steps in care. If you’re a parent, a yearly child checkup can also count as a sports physical, but a sports physical is not a substitute for a checkup.

Women's Health

Cardiac Rehabilitation
Cardiac rehabilitation treatment sessions can help you recover from a heart attack, disease or surgery. Pulmonary Rehabilitation
Pulmonary rehabilitation treatment sessions help improve quality of life for those diagnosed with lung conditions. The care team evaluates and instructs patients on physical activity, nutrition, stress management and medication education, along with meeting other educational needs for patients.

Occupational Medicine Services
Many local businesses require employment screenings. Some jobs require regular physical evaluations to maintain certifications to operate equipment. Our clinic in Granite Falls can accommodate many common employer- and employee-related services and requirements.

Anticoagulation Management
This clinic offers anticoagulation management services. Anticoagulation management is the patient-physician process of overseeing and adjusting blood-thinning therapy to prevent blood clots while minimizing the risk of bleeding. It also involves selecting the appropriate medication, educating the patient, and regularly monitoring and adjusting treatment based on individual needs and risk factors.

Sage Screening Program
We offer the Sage Screening program that helps keep Minnesotans healthy through screening and early detection of breast, cervical and colorectal cancers. Screenings are provided at participating locations free of charge to those who qualify.
The Sage Program maintains a call center, staffed by patient navigators who speak several languages. The navigators answer calls during normal business hours and assist Minnesota callers with program eligibility, identifying and addressing barriers to screening, scheduling appointments and providing referrals to appropriate resources.

Reach Out and Read Program
To connect with our youngest patients, we implement the Reach Out and Read Program. During a well-child visit, our clinicians utilize reading and books to help children learn a little bit about their health and the care they’re receiving.
