For over 50 years, Southwestern Mental Health has provided care for our communities. Our services include the following:
Insurance Information
As a National Health Service Corp site, Southwestern Mental Health Center promises to accept all Health Insurance plans, Medical Assistance, Medicare and other forms of third party payments. Residents of our five incorporating counties (Cottonwood, Jackson, Nobles, Rock and Pipestone) who do not have insurance may be eligible for adjusted fees based on family size and income. Southwestern Mental Health Center, Inc. does not deny services based on a person’s ability to pay, race, color, sex, national origin, disability, religion or sexual orientation.

Adult Residential Mental Health Services
Unity House is a 10-bed residential facility located in Worthington, MN. Unity House is licensed to treat adults with serious and persistent mental illness. Unity House services develop and enhance psychiatric stability, personal and emotional adjustment, self-sufficiency, and skills to live in a more independent setting.
Mobile Crisis Response Team
Our Mobile Crisis Response Team is available 24 hours a day, seven days a week to anyone experiencing a mental health crisis. Since the team is mobile, we can meet a person at home, school, work or wherever a crisis occurs. Crisis services are available at no cost to the recipient. Call the 24-hour Crisis Helpline at 1-800-642-1525, or during office hours at the Windom, Jackson, Worthington, Luverne or Pipestone offices.

Southwestern Mental Health Center, Inc. is a private, non-profit agency, established in 1959 by Cottonwood, Jackson, Nobles, Pipestone and Rock counties in Southwest Minnesota. We’re licensed by the Minnesota Department of Human Services. Our fully qualified and credentialed staff offering a variety of mental health services.
Fee is based upon catchment area residents on their ability to pay. No one is refused proper care based on his or her ability to pay, race, creed, ethnicity or background.
