Wagner Community Memorial Hospital Avera, located in Wagner, SD, is a 20-bed Critical Access Hospital equipped with advanced services, including a medical/surgical area, pediatric care, and swing bed/intermediate care. The Wagner Community Clinic is adjacent to the hospital making it convenient to see hospital patients, share laboratory services and other diagnostic equipment. Our providers are certified in Advance Trauma Life Support (ATLS) and emergency room medicine. The nursing staff is ACLS and TNCC certified. With the addition of the eEmergency we have the ability to visually and audibly connect with specialists in Sioux Falls.
Wagner Community Memorial Hospital Avera is dedicated to community education. We educate local schools and the local community by holding health fairs as well as a local career fair. We participate in the community's Farm, Home, & Garden Show. We provide education on a variety of health prevention and medical topics including diabetes, stroke warning signs, drug use prevention and healthy lifestyle tips. We also have CPR classes available. Wagner Community Memorial Hospital Avera offers a special program for continued care beyond an acute hospital stay. This program, called Skilled Swing Bed, allows the patient a short-term stay in the hospital before returning home to recuperate or make alternative arrangements.
The Skilled Swing Bed program is typically used for rehabilitation after an illness or surgery and for intravenous therapy or medication. Patients who may use the program include those recovering from a stroke or accident, people who need IV medication and wound care.
Skilled care in the Swing Bed program requires the daily skills of qualified health care professionals, such as registered nurses, physical, occupational, or speech therapists.
Intermediate long-term care is also used as a recuperation period, when the patient requires a little extra care and does not feel like they are strong enough to go home. This is also a short term stay, but does not require any skilled services such as Physical therapy or skilled nursing. Intermediate long-term care is usually private pay, although long-term care insurance may cover some of the costs.

- All co-payments are due and payable at the time of service.
- If you receive care for a work related injury, notify the receptionist when you arrive for your appointment.
Help us submit your claims to your insurance company, Medicare or Medicaid by giving our business office accurate insurance policy information. You’re also responsible for negotiating with your insurance carrier.
