Avera MyChart

On May 30, 2026, Avera will transition to a new patient portal, Avera MyChart. MyChart gives you one secure place to view your Avera health information. MyChart will be available with limited features starting May 18-29 for select patients and health plans members with specific needs. After May 18, you may receive an email or text message from Avera as a patient or member with a request that you sign up for MyChart.

  • See current prescriptions and request refills.

What Happens to the Current Portal

  • Starting May 14 at 5 PM, you can only make, adjust or cancel appointments by calling the clinic.
  • Starting May 29 at noon, provider messaging and refill requests will be unavailable in the legacy AveraChart portal. Contact your pharmacy if you need a refill during that time.
  • May 30, the AveraChart portal will become view-only access to look at information. Most information will transition to your new Avera MyChart portal.

What You Need to Do When MyChart is Live

You don’t need to do anything just yet. We’re sharing details now, so you’re prepared to get started once the portal is available May 30.

1. Register for MyChart

Current patients and new patients will need to create a new account on or after May 30. Your old account login and password will not transfer to the new patient portal.

2. Reestablish Proxy Access

If you have access to another person’s health information for coordinating care – such as if you have a child or if you help manage your parent or another adult’s health appointments – you will need to reestablish this access in MyChart.

We will provide instructions and forms to make this process as easy as possible when MyChart launches.

What You Need to Know

What happens to my current patient information in AveraChart?

Your past health records will transfer to Avera MyChart. Any scheduled appointments we have will also be transferred and available to see in MyChart after May 30 (once you register).

One thing that will not transfer is proxy access information. If you currently manage care for family members or others, you will need to reestablish proxy access in MyChart. Watch for more information and instructions soon.

What happens to my already scheduled appointments?

This information will transfer. After May 30, you will be able to see these appointments and manage them in MyChart. Note that some previous patient information may not be available until your next visit.

What if I already have MyChart with another health system? 

After you create your Avera MyChart account, you’ll be able to link other MyChart accounts, so you can access them from the one app.

Will I need to create separate logins for Avera and Avera Health Plans?

No. If you’re an Avera Health Plans member, you’ll be able to use one login to see both Avera Health and insurance information in the portal. Learn more about MyChart and Avera Health Plans.

Will there be a mobile app?

Yes. You’ll be able to log in to MyChart via a web browser or through the Avera MyChart app, which will be available for both Apple and Android devices.

Is there a fee to use MyChart?

No, Avera MyChart is free to use for Avera patients and Avera Health Plan members.
