Our patient and member portal gives you a secure way to access your health information and interact with your care team, as well as manage your health insurance if you’re a member of Avera Health Plans. In MyChart, you can:

  • Schedule care easily online or in the app.
  • Connect with providers with virtual visits.
  • Manage care for family members.
  • Pay premiums and see medical bills.
  • See current prescriptions and request refills.
  • Access your digital insurance card and manage insurance coverage (if you have Avera Health Plans insurance).

Who can use Avera MyChart?

Any Avera patient (or Avera partner/affiliate facility) age 14 or older is eligible to enroll in Avera MyChart. Parents or guardians can request proxy access to a child’s health information. Access depends on the child’s age:

  • Children ages 13 and younger: Parents or legal guardians can request proxy access and view the child’s full record.
  • Teen ages 14-17: Teens can sign up for their own Avera MyChart account. Parents or legal guardians can request proxy access, but access to some information may vary by state law based on the state where the teen is a patient.

What happens to my current patient information in AveraChart?

Your past health records will transfer to Avera MyChart. Any scheduled appointments will also be transferred and available to see in MyChart after May 30 (once you register).

Account setup and proxy access are not transferred to Avera MyChart. You need to register for an Avera MyChart account and reestablish proxy access if you currently manage care for family members or others.

Patients who previously had an AveraChart account will have temporary, read-only access to the legacy patient portal for a limited time.

What happens to my already scheduled appointments?

This information will transfer. After May 30, you will be able to see these appointments and manage them in MyChart.

What if I already have MyChart with another health system?

After you create your Avera MyChart account, you’ll be able to link other MyChart accounts, so you can access them from the one app. This helps give you a complete picture of your health and is especially useful if you see specialists or primary care providers at separate hospitals or clinics.

  1. Log in to MyChart.
  2. In the Menu, go to the Sharing section and select Link My Accounts.
  3. Search for the health organization where you have a MyChart account. Select Link account next to that organization.
  4. When prompted, enter your login credentials for your other MyChart account.

Once your accounts are linked, you’ll be able to toggle between health organization accounts using the dropdown on the top right.

Note: If you are an Avera Health Plans member, you still need to download the Avera MyChart app or use AveraMyChart.org to see and manage all your health plan benefits.

Will I need to create separate logins for Avera and Avera Health Plans?

No. If you’re an Avera Health Plans member, you’ll be able to use one login to see both Avera Health and insurance information in Avera MyChart. Learn more about the portal and Avera Health Plans.

Can I use a mobile app for MyChart?

Yes, in addition to using the AveraMyChart.org website, you can also ownload the Avera MyChart mobile app, available in the Apple App Store and on Google Play.

If you receive care from an Avera partner location, you can either use the Avera MyChart app or the general MyChart app. However, if you’re an Avera Health Plans member, your insurance coverage details are only available in Avera MyChart. Learn more.

Is there a fee to use MyChart?

No, Avera MyChart is free to use for Avera patients and Avera Health Plans members. There are charges, however, associated with various Avera and Avera Health Plans services.

How is Avera MyChart kept secure?

Keeping your health information private and secure is a top priority at Avera. We use the latest available technology to ensure your data is protected, including two-step verification, secure activation codes, face ID and fingerprint login options for MyChart.

Requiring two-step verification helps ensure your account stays secure even if someone else has your username and password. How it works:

  1. You enter your MyChart username and password on a login screen.
  2. You’ll receive a code via email or text message that includes a single use code.
  3. Follow the steps to complete authentication into your account.

If you requested a verification code via email and did not receive it, check your spam or junk folders for your email – and if you have multiple email accounts, check them all.

If you think your email or phone number where you receive Avera MyChart notifications and secure links has been compromised, you can change them in MyChart under Personal Information.

What are the past patient portals for Avera, and can I still use them?

Most legacy portals are only available for view-only access for patients or clinicians, and they may be sunsetted in the future. They each have had their own unique logins and are not accessible by using your MyChart username and password.

  • AveraChart – This portal is available for existing patients with accounts to view only starting May 30 until December 2026.
  • Oncology & Hematology Patient Portal, powered by Kaiku – Access expired for patients when MyChart launched on May 30, 2026.
