Have Avera Health Plans for Insurance?

The new Avera MyChart is a single portal for both Avera patients and Avera Health Plans members, so all of your information is in one place with one login.

  • You have a new member ID and prescription ID number, which has been automatically updated at Avera clinics and pharmacies.
  • For non-Avera clinics and pharmacies, you can show your digital ID card in your app.
  • You can see and take action on much more about your Avera Health Plans insurance coverage.

Have Another Insurance Provider?

In Avera MyChart, you’ll see basic details that our care team has on file for your insurance policy. If you have a new policy or provider, you can add this information in MyChart or provide it when you visit an Avera care location.

Add Your Insurance in MyChart

  1. Go to the Insurance section in Avera MyChart. (You’ll need to log in first.)
    • If you’re already logged in and want to navigate to Insurance, select the top left menu icon. Scroll or search for Insurance. Select the Insurance Summary menu option.
  2. Select Add a coverage.
  3. Choose an insurance provider in the dropdown menu.
  4. Enter your policy details and upload images of the front and back of your insurance card.
  5. Select Submit.

View or Edit Your Insurance on File

  1. Go to the Insurance section in Avera MyChart. (You’ll need to log in first.)
    • If you’re already logged in and want to navigate to Insurance, select the top left menu icon. Scroll or search for Insurance. Select the Insurance Summary menu option.
  2. You will see the details we have for your insurance on file. Here, you can select one of these options:
    • View coverage details: See more information that we have on file for your policy.
    • Update coverage: Enter comments about coverage changes and click Submit.
    • Replace insurance card photos: Upload images of the front and back of your insurance card. Click Submit.
    • Remove coverage: Add optional comments and click Remove.
