Partner Sites
A number of hospitals and care locations in the upper Midwest have partnered with Avera to offer MyChart to their patients. These organizations have different business relationships with Avera, but all of them use MyChart as an extension of Avera’s technology. Some of them also have their own websites.
MyChart for Partner Sites
You can use the MyChart website for all partner locations or download the MyChart mobile app, available in the Apple App Store and on Google Play.

- If you’re a patient of both Avera and a partner location, you can use the partner site MyChart or Avera MyChart. Your medical information for both organizations will show altogether.
- If you’re a patient at a partner location but also use Avera Health Plans, you will need to use Avera MyChart to see your Avera Health Plans coverage and insurance information.
