If you previously used AveraChart or another patient portal with Avera, your medical information is transitioning to MyChart; however, your login account details are not. You must set up a new MyChart account.
MyChart enables you to easily manage your care, message your provider, have access to your medical records, pay bills and much more.
The MyChart access code is a code that’s custom to you that enables you to sign up for Avera MyChart in just a couple steps – especially if you have already been a patient or member with Avera or one of our affiliates.
If you’re new here, we may require additional information when you register for an account.
For security and privacy reasons, MyChart access codes expire after a certain amount of time. If your access code is lost, expired or you didn’t receive one, try these options:
- Recommended: Contact our help desk at 855-667-9704 for a new code to streamline your registration.
- Contact your clinic and ask them to generate a new code and resend it to you.
- Register for MyChart without an access code. (This requires you to fill out more demographic information than if you were already in our system and had a code.)
When trying to use the MyChart self-signup option (without an activation code), some people may not be able to sign up. As a result, they will receive a web page message and an email that we were unable to verify their information and that their request is being processed. This occurs because we were unable to directly match the information you submitted (such as your email address or phone number) to an existing patient record. For privacy security purposes, we want to make sure the right people have access to the right patient and member records.
What to Do
You have three options:
- Wait for us to send your activation code via email or mail (within one week).
- Try to register again using the email address and/or phone number that you know is already on your patient record with us.
- Call us 24/7 to get your activation code from the MyChart tech support team at 855-667-9704.

Yes, you need a current email address for MyChart access. If you don’t have an email address, you can get your medical records by completing our medical records request form.
Teenagers aged 14-17 can enroll in their own Avera MyChart account to manage their care. With MyChart, teens can schedule appointments and take part in a video visit, join waitlists, send messages to their care team, manage prescriptions, and view test results and other health information.
- Teens must create an account on their own. Parents should not create one for them.
- By law, parents and legal guardians will automatically have access to the children’s medical record, with some differences by state.
- Access is not allowed for parents of emancipated minors.
- There are various other relationship categories to consider in relation to medical record access.
