You can request your Avera medical records for yourself or share them with another health care entity or provider. Records are released in accordance with state and federal laws.

Request Online
Use Avera MyChart to request your patient medical record for multiple uses. You’ll need to log in (or create an account) to submit your request online. The MyChart Sharing Hub is the easiest and fastest way to view and share information such as visit summaries, test results, medications, immunization records and more.

Common Reasons to Share Your Medical Record
There are few different scenarios when you might want to request or give others access to your medical records:
- For yourself: Download or request a copy of your full medical record for your own needs.
- For a family member, close friend or caretaker: This is often a parent or adult child who is helping you track your health. They can get proxy access to your record in MyChart.
- A health care provider: This could be another health care organization or a provider such as a doctor, dentist, nurse or social worker.
- Anyone else: This could help you provide necessary health information with someone such as a school nurse.
Get Your Medical Records Online
In the Sharing Hub within MyChart, there are helpful videos demonstrating the steps to take to share your health record. You can start by selecting who you want to share your health information with and following the on-screen options – or go to the bottom of the page for some common ways people share, such as:
How do I share my medical records with another health care provider?
- Go to the Sharing Hub in Avera MyChart. (You’ll need to log in first.)
- If you’re already logged in and want to navigate to the Sharing Hub, select the top left menu icon. Find and select the Sharing Hub menu option.
- On the Sharing Hub page, select Healthcare provider.
- Select one of these options:
- Download or send a snapshot: This option makes your medical record available immediately.
- Enter the date range for care and then choose Download or Send the file(s).
- Enter the requested information to complete the download or to send the file(s) to a provider, direct address or to an email address.
- Request a formal copy: This request requires processing by us and might take a few days.
- Answer the on-screen questions for who should receive the record, the date range for care and what information to include in the record. Select Continue.
- Review the summary of your request, and choose I agree to give your consent to release the information. Choose Send request.
You will see a list of Requested Records that you’ve submitted in MyChart. When the request is complete, you may receive a notification from MyChart (depending on your communication preferences).
How do I access medical information to share for school health purposes?
- Go to the Sharing Hub in Avera MyChart. (You’ll need to log in first.)
- If you’re already logged in and want to navigate to the Sharing Hub, select the top left menu icon. Find and select the Sharing Hub menu option.
- Select Download School Health Summary.
- Review the disclaimer and check the acknowledgement box. Select Request download.
How do I connect MyChart to a third-party app, such as one that tracks my health?
- Go to the Sharing Hub in Avera MyChart. (You’ll need to log in first.)
- If you’re already logged in and want to navigate to the Sharing Hub, select the top left menu icon. Find and select the Sharing Hub menu option.
- On the Sharing Hub page, select Anyone else and then Give a third party app access.
- A new page will appear to explain how to start this connection process through your third-party app in order to allow a connection to MyChart.
Medical Records FAQ
When I share my medical record with another provider using a share code, what can they see?
The person you send the share code to can see some of the same information you see in your own MyChart account, such as:
- Medications, allergies, health issues and medical history
- Immunizations, lab results and tests
- Visit records, gaps in care and code status
If part of your chart isn’t available to in MyChart, it won’t be available to others through sharing either.
How can I see where a list of medical records I’ve requested or downloaded?
- Log in to Avera MyChart.
- If you’re already logged in and want to navigate there, select the top left menu icon. Find and select the Document Center menu option, which is located under My Record.
- On the Sharing Hub page, select Requested Records. If you’ve made a request, it should show on the Requested Records page.
