Custom Home Care Plans

Home health care plans are tailored to help patients be well and manage their condition at home with as much independence as possible. The transition of medical care from a facility to home occurs within 24 hours after discharge.

Dedicated Care Team

Certified medical professionals, nurses and support staff make up your home health care team.

  • Home health aide: Provides personal care services and help with daily living activities, such as bathing, housekeeping and grocery shopping.
  • Home health nurse: Assesses your health and provide services, such as medication, IV and nutrition support, diabetes education, blood draws and care for wounds, ostomy and catheters. Home health nurses also help patients experiencing high-risk pregnancy and provide care for mothers and newborn babies.
  • Occupational therapist: Assesses and develops plans for activities and everyday tasks to help patients return to their work and lifestyle.
  • Pharmacist: Provides advice, education and support to patients regarding managing prescribed medications.

Hospital@Home in & Around Sioux Falls, SD

Instead of staying at Avera McKennan Hospital & University Health Center, some patients may receive ongoing hospital treatment and evaluations at home. Your doctor will review your situation and will determine if care can be safely administered at home. Our team helps facilitate that support and care.

Eligibility

Patients must:

  • Be able to be safely treated at home with frequent checkups
  • Live within or stay with family within 30 miles of Sioux Falls
  • Need hospital-level care

If a patient needs more advanced care, they will be transferred back to the hospital.

Services

Your doctor works closely with hospital staff to ensure all your medical needs are met. Hospital@Home services to help may include:

  • Daily assessments and check-ins
  • Medical equipment brought to patient as needed
  • Medication management
  • Multiple nurse visits per day as needed
  • Virtual visits with doctor or provider

  • Don’t have to wait for hospital discharge since they are already home.
  • Have fewer health care staff visit them, making the experience more personal.
  • Sleep in their own beds with no interruptions at night (unless requested by the patient).
