Dedicated Care Team
- Home health aide: Provides personal care services and help with daily living activities, such as bathing, housekeeping and grocery shopping.
- Home health nurse: Assesses your health and provide services, such as medication, IV and nutrition support, diabetes education, blood draws and care for wounds, ostomy and catheters. Home health nurses also help patients experiencing high-risk pregnancy and provide care for mothers and newborn babies.
- Occupational therapist: Assesses and develops plans for activities and everyday tasks to help patients.
- Physical therapist: Focuses on providing hands-on therapy and exercise treatments. They design personalized movement programs to improve mobility, strength, endurance, coordination and balance.
- Speech therapist: Supports patients needing help with facial muscle therapy for talking, eating and swallowing, plus the ability to communicate with others.
