Coordinated, Compassionate Home Care

Take advantage of coordinated in-home services from Avera@Home—no matter what your age or where you live in the Upper Midwest. You can receive overall evaluation of your health and living situation, companionship and education about health conditions and self-care, as well as assistance to:

  • Make a transition from hospital to home
  • Manage medications
  • Enroll in support programs to help you stay in your home
  • Regain the ability to perform everyday activities through physical therapy
  • Address in-home access and safety issues with occupational therapy
  • Change bandages/dressings, manage wound care and IVs or central lines
  • Support newborns at high risk or people of any age with significant issues
  • Handle light housekeeping, shopping, meal preparation and personal care

In-Home Care for Faster Recovery

Recover from illness or injury as quickly as possible through services delivered in the comfort of home. You’ll appreciate collaborating with Avera’s experienced professionals who address a wide range of concerns. Therapeutic services include:

  • Physical therapy – Helps increase strength, improve mobility and balance, relieve pain, prevent or limit physical disability, restore physical function and self-sufficiency to the highest possible level for your specific situation
  • Occupational therapy – Evaluates and adapts your environment to promote independence, helps you recover or maintain the skills of daily living and work, and enables greater participation in a meaningful and satisfying life
  • Speech-language therapy – Supports return to function for speaking, thinking, understanding, reading, writing, swallowing and communication-related issues

Your Home Health Care Team

You and your doctor—who oversees your care—work with several Avera home health team members, including:

  • Skilled nursing care staff – Assess your health and deliver a wide range of services, such as medication, IV services and nutrition support, diabetes education, blood draws, wound/ostomy/Foley catheter care, high-risk pregnancy and aftercare for mother and newborn
  • Social workers – Assist you and your family members in coping with challenges related to illness, disability and terminal conditions; evaluate problems, needs and coping mechanisms; connect you with community agencies, education programs, counseling services and support resources
  • Pharmacists – Deliver advice and support regarding medications
  • Dietitians – Offer nutritional guidance and support
  • Home health aides – Provide companionship and help with bathing, grooming, personal hygiene, meal preparation, grocery shopping, light housekeeping, laundry, personal correspondence, and getting in and out of bed or chairs
  • Specialists – Deliver in-home physical therapy, occupational therapy, speech therapy or other specific therapeutic services
  • Therapists – Provide physical therapy, occupational therapy and speech-language therapy
