Basic Metabolic Panel* - $10
The basic metabolic panel (BMP) typically involves eight separate measurements and can provide relevant information in many situations depending on the context. The BMP can be used for medical screening, diagnosis or monitoring treatment. Panel components: sodium, potassium, chloride, CO2, glucose, blood urea nitrogen (BUN), calcium, creatinine.
Blood Typing (ABO + Rh) - $25
Determines your ABO and RH blood type. Blood typing is a common test when blood transfusions and tissue transplants are required as well as during pregnancy.
C-Reactive Protein - $10
C-Reactive Protein (CRP) is a protein produced by the liver released into the bloodstream within a few hours after the start of an infection or inflammation. CRP levels are also used to diagnose and monitor autoimmune disorders. Increased CRP levels are found in conditions including: bacterial infection, rheumatic fever, active arthritis, myocardial infarction, malignancies and in the post-operative state.
Celiac Panel - $90
A celiac panel may be used to diagnose celiac disease. Total IgA is performed and reflexed to additional tissue transglutaminase antibodies based on results.
Chicken Pox Immunity - $20
Testing is used to diagnose a recent or past case of chickenpox or shingles. This test determines immunity to the Varicella Zoster virus via infection or vaccination.
Complete Blood Count with Differential - $10
The complete blood count (CBC) is an essential comprehensive blood panel used to evaluate each type of cell in the blood. This test helps determine general health status including anemia, infection or inflammation.
Complete Metabolic Panel* - $15
A comprehensive metabolic panel (CMP) is a blood test with 14 different measurements. It is often used to evaluate liver and kidney function as well as nutrient levels. The panel includes: sodium, potassium, chloride, CO2, glucose, albumin, total protein, calcium, total bilirubin, creatinine, alk phos, ALT, AST and blood urea nitrogen (BUN).
Estradiol - $35
Estradiol is one of the main types of estrogens and plays a role in both females and males. This test may be used for assessing hormone levels and monitoring hormone therapies.
Ferritin - $15
Ferritin is a protein that stores iron, necessary for red blood cell production and the distribution of oxygen throughout the body. When the body needs iron, a small amount of ferritin is released from cells and circulates in the blood. Ferritin levels reflect the total amount of iron stored in the body.
Folate (Folic Acid)* - $20
An inadequate level of folic acid can lead to anemia and other changes to health. Anemia may cause symptoms such as weakness, shortness of breath and mental changes.
Follicle Stimulating Hormone - $25
Follicle-stimulating hormone (FSH) is made by the pituitary gland and control of its production is a complex system involving the hypothalamus, pituitary gland and hormones produced by the ovaries or testes. This test can be used to evaluate fertility, the health of reproductive organs, or pituitary function.
Free T3 - $20
Triiodothyronine, known as T3, is an important thyroid hormone that regulates metabolism. Free T3 measures the active form of T3 that is not bound to protein. This test may be ordered to evaluate a thyroid disorder.
Free T4 - $10
T4 (thyroxine) is the main form of thyroid hormone made by the thyroid gland. Most T4 is bound to protein while a small portion is unbound or free. Free T4 testing measures T4 that circulates through the blood and is available to enter body tissues and act upon them.

Hepatitis B surface antigens are proteins present on the surface of hepatitis B virus which can be detected in high levels during both acute and chronic hepatitis B infection.

Thyroid Panel - $35
Testing is used to screen for thyroid dysfunction and monitoring thyroid replacement therapy. Panel components include: TSH and FT4.
Thyroid Stimulating Hormone - $25
Evaluates the thyroid gland function and monitors treatment. Testing measures the level of thyroid hormone in the blood.
Tuberculosis Screen (TB Quant) - $110
TB blood tests, or interferon-gamma release assays (IGRAs), are used to check for TB infection.
Uric Acid - $10
This test is used to detect high levels of uric acid to diagnose gout or kidney stones. It also monitors levels for those undergoing cancer treatment.
Vitamin B12 - $20
Having a vitamin B12 deficiency, or a low level can cause a range of health issues. Adequate levels of vitamin B12 are needed to maintain a healthy nervous system, make red blood cells and create DNA.
Vitamin B12/Folate Panel* - $40
Vitamin B12 and folate tests measure levels in the blood to help identify potential vitamin deficiencies. B12 or folate deficiency may lead to fatigue, weakness or anemia.
Vitamin D (25, Hydroxy) - $35
This is the preferred test to diagnose vitamin D deficiency and monitor response to therapy.
All testing is done by Clinical Laboratory Improvement Act (CLIA) federally licensed laboratories.
Laboratory test results, with the exception of Genefolio Vitality, will be available within hours for you to view in the AveraChart patient portal and the Avera Lab Direct Access Testing Portal.
In order to receive the most accurate results, some tests might require fasting – or refraining from eating and/or drinking certain foods for a set amount of time. A * symbol in the test name indicates testing in which fasting is recommended for 12 to 14 hours.
Payment is due at the time of service. Insurance, Medicare and Medicaid will not be billed for Avera Lab Direct Access Testing services.
