The Floyd Valley Foundation works with caring donors who contribute financially to benefit patients of Floyd Valley Healthcare.
Because Floyd Valley is a non-profit entity and our operations are not funded by local tax dollars; gifts from our friends, former patients and their families can make a huge impact. Generous donors help to improve facilities, update technology, expand services and impact the lives of many.

Ways to Give
Choose a giving option that’s most convenient for you. However you give, we appreciate your support.
1. Mail a check:

Please indicate where you would like your gift designated in the memo line of your check or in a separate note. Make sure your contact information is correct on your check and include your telephone number in case there are questions.
You can donate by check or cash as a memorial or in honor of a friend or loved one. Include your check to the Floyd Valley Foundation and place it in the sympathy card that you give to the family, or mail to the address above.
With either method, write in the memo line the name of whom the gift is in memory of. The Foundation will notify the family of your gift.
2. Set up automatic payments
To authorize automatic payments directly from your bank account, download and complete the authorization form, fill it out, and return it with a voided check to the provided address. This option is a convenient for you while providing extra savings by reducing processing fees. The form also allows you to designate your gift and provides opportunities for monthly, quarterly and perpetual giving.
3. Donate online

Floyd Valley staff may elect to have gifts deducted from their paycheck. To set up a new payroll deduction or edit an existing one, please download the staff pledge form.
4. Give by phone

More and more people are making donations through the following methods because they find them practical and affordable, and they may provide income and estate tax benefits:
• Donation of retirement plan assets
• Donate gifts of stocks, mutual funds, property, or real estate
• Donate gifts of grain or cattle
When you establish a charitable gift annuity or charitable remainder trust, you receive income from these investments.
Remember the Floyd Valley Foundation in your will or estate plans, or make the Floyd Valley Foundation the beneficiary of a life insurance policy.
